Two questions
Posted 26th Feb By Eithne Murray
I am looking for that photograph of Jackie Kent's (I think) of the 10 test tubes of breastmilk taken at different times in the feed. It was in one of the Breast-ed courses, but I've foolishly lost it.
What publication was it in originally? Thanks.
And there was also an interesting quote of Lisa Amir's about the difference in colour in the nipples between thrush, vasospasm and staph infection. Can't find that either.
I've looked at length for both of those and can't find them. Glad I'm not doing the exam this year.

Oh Eithne I'm sorry I've taken so long to respond to this - it completely slipped my memory.
The test tube photograph demonstrating gradually increasing fat levels during a breastfeed was supplied to me personally by Jackie. I don't know of it being published anywhere else. You could email Jackie and she may be happy to provide you with a copy.
I'm not sure which Amir quote you are referring to ... could it have been something that was written in the forums.
These are the two Amir articles that are referenced in the 'nipple infections' topic: Amir LH, Garland SM, et al. (1996) Candida albicans: is it associated with nipple pain in lactating women? Gynecol Obstet Invest 41(1):30-4
and
Amir LH, Lumley J, Garland SM. (2004) A failed RCT to determine if antibiotics prevent mastitis: Cracked nipples colonized with Staphylococcus aureus: A randomized treatment trial BMC Pregnancy Childbirth 4:19
I hope this provides the answers you need, and sorry I couldn't be more helpful.

Thanks for that denise. I've managed to locate the photo and been in contact with Dr Kent. She is wonderfully generous with her time.
Re Amir: I recall someone on this list I think, saying they had heard Amir, in person, give a useful shorthand way of using colour to distinguish between the different types of nipple soreness. It went something like:
red/pink/white/ yellow. The last 2 were for vasospasm/nipple infection.It's the others I can't remember, and can't find.

I'll ask the BE07 tutor if she can locate the forum posting.

Eithne, I went straight to the top - I asked Lisa Amir directly.
Lisa presented her "guide to nipple colour" as part of a presentation on nipple pain.
She has allowed me to pass it on, with copyright to her. She stresses that this is a guide only. No photos tho.
Pink/Red - candidia
Yellow - staph
Bright Red - eczema, dermatitis
White - vasospasm.
I would wholeheartedly support these observations as a starting point for diagnosis.
The big confusion is misdiagnosing dermatitis for thrush. A reminder not to persist with antifungals that are not working. Even severe thrush will show some signs of relief within 48 hrs of treatment.
breast wishes
