therareutic ultrasound for blocked ducts

hi denise,
i have a mother with a 4 week old baby. she has a huge lump in her left breast (retroareolar region) which has pulled her nipple in, thus making it impossible for the baby to feed on that side. she also has a small lump in right breast (in upper outer quadrant) ,but can comfortably feed from that side. usg shows these to be galactoceles. she has been massaging and pumping for 8 days now, but there is no difference. she is on antibiotics and lecithin capsules.is there any special technique of massaging over the lump?

should i try therapeutic ultrasound therapy for? will it help her to soften the lump and express out the accumulated milk? do you have any personal experience regarding this? how do you manage such patients?

Hi Preeti

Have you tried needle aspiration? There's quite a few journal articles published that you could find in PubMed about it, particularly as it is being used to drain abscesses, avoiding surgery.

There was a randomised double blind trial done on therapeutic ultrasound some years ago which found it to be no more effective than massage and warmth.
